Yorkshire Community Care – Working with Solicitors and Case Managers

Whatever organisation you work for, as a case manager your job is only made possible by the reputable home care providers you collaborate with.
At Yorkshire Community Care, we understand the importance of finding home care specialists you can rely on. We have worked with several funding bodies, private organisations and individual case managers to deliver the very best solution for the clients that they care for. With our help you can strike the right balance between finding a home care partner that’s dependable and flexible and recruiting a professional that’s great value for money.

How we work with case managers

Our team is as passionate about providing and facilitating the highest standard of outcome-based care as you are. We provide the full package, meaning case managers can access the assistance they need to deliver the best possible care to their own clients.

Yorkshire Community Care’s specially trained and experienced advisors deliver case management support, and advise on, organise and implement domiciliary and live-in care packages for clients based up and down the country. From local hubs across the UK, we work directly with case managers to provide the personalised, professional service needed to establish and cater to the unique needs of each and every member of their client base.

We have experience assisting care case managers at the pre-settlement stage, with initial package costs and conditions for care available from our team to support litigation. Efficient and effective support therefore comes as standard, and we actively relay quotations in a comprehensive and fair manner whatever your needs. Before coming to a decision on costs and conditions, detailed assessments are conducted either via telephone or in person.

Are you a case manager looking for assistance with a post-settlement case? We can help. Again client needs are established through assessment and a tailor-made care package built with timescales and measures that are personal to them. Their bespoke package is then implemented with the help of our local managers and carers, who continue to collaborate with case managers to ensure the best outcomes can be realised every step of the way.
Robust, long-term care planning is what we do here at Yorkshire Community Care. Ourselves and the dedicated staff who work for us are committed to maximising quality of life and independence. They work closely with case managers to make both possible for their clients. As part of our promise to you, plans are reviewed regularly to make sure these are actively achieved.

Complex Case Management Support – Care Case Assessments

We understand that everyone’s needs are personal to them. As a result, care case assessments and ongoing reviews are important parts of what we do. Through detailed assessment, we’ll evaluate the physical, emotional and social requirements of each client and go onto draw up a bespoke individual detailed support plan (DSP).

Our clinical nurse managers are in charge of conducting these assessments. They are also supported by our local care managers to ensure the level of specific condition-based care exceeds expectations. Visits are an integral aspect of the assessment process.

Our work doesn’t end there. With the help of our findings, we can recommend and arrange specialist equipment and adaptations that will support the care package. We’ll also choose a carer with the right training and personality to meet the individual needs of each client. Final costs and budget forecasts are a part of our comprehensive service to you, with our accounts department specialists in the delivery of effective and efficient financial administration.

Here at Yorkshire Community Care, we understand that as a case manager there’s no ‘one-size-fits-all’ for your clients. Real people have different needs and varying personalities that have to be catered for. Each of our complex care packages are devised with this in mind. We work with case managers and solicitors every step of the way to ensure we deliver care that is in tune with the person.

Our professional care networks are transparent and built on a foundation of quality, experience and skill. All records are kept up to date and reviewed regularly to ensure the outcome-based care solutions we aim to supply are delivered for the long term. We pride ourselves on our positive working relationships, where care staff aren’t just a part of the team but a part of the family.

Outcome-Based Care Solutions

Outcome-based care matters, especially in the delivery of our care case management service. The tailor-made care packages we develop are implemented by Direct Support Professionals (DSPs) who are not only passionate about what they do but qualified and experienced. Person-centred care is what Yorkshire Community Care is all about, with the unique requirements of your clients used to deliver an effective plan that sees significant improvements to their quality of life.

Our people are your people, that’s why we’re happy to share the skills, experience and backgrounds of all our carers before a package of care is even proposed. Each carer is selected to meet the needs of your clients. Through meticulous matching, we can pair your client with a carer that has the nursing-led qualifications, understanding and character that complements them.

Tailored Medical Assistance

High quality comes as standard throughout our care packages. Through detailed assessment we can devise a tailored support plan from the very beginning. We have experience delivering tailored medical assistance, and are certain we can cater to the requirements of any client, whether they need live-in support, help outside the home or visiting care.

We are well-versed in meeting the complex and social care needs of a vast range of clients, with our carers providing active support to people with a number of conditions, including those with multiple sclerosis, cerebral palsy, spinal injuries, acquired brain injuries and motor neurone disease. Clients requiring bladder and bowel management, PEG feeding and management, insulin administration, BiPAP and CPAP care, continence care, and single and double-up hoisted transfers can also find the tailored medical assistance to live well in their own homes.

Once a plan for medical assistance has been carefully devised, it is managed and monitored to ensure our healthcare and multidisciplinary teams (MDTs) continue to deliver the excellent service they set out to supply. Our staff take a proactive approach to observing and reporting any change to client needs, and associated care plans are adapted accordingly.

Care Case Management – Continuity

We’re here for your clients for however long they need us. All our carefully devised care packages are flexible, and able to be adapted to any changes in condition or needs. Guaranteeing continuity in care is important to us and where possible we assign a named specialist to each case to ensure the highest standards are delivered every step of the way. When this named carer is on leave, seamless cover is provided with 24-hour handovers a standard part of our procedures.

Local care managers play active roles in ensuring continuity in care case management. These specialists visit and call clients on a regular basis to ensure any ideas or concerns can be raised and addressed promptly. As a case manager you will work closely with our local care managers, who each has a solid background in implementing long-term, people-centred care packages throughout the country.

We work seamlessly with case managers, solicitors and other health care experts to ensure quality in care, independence and dignity are achieved for their clients.
